Members of the Medical Staff can
obtain permission to enter the Medical Staff Web Site by requesting a username
and a password. To request a password:
1. Click the "Request A Password"
link at left or at right
2. Complete the required information
3. Submit
your information by clicking the "Create my account..." button
4. You will
receive an e-mail that your request for approval has been submitted
5. You
will receive another e-mail when your request has been approved
6. After
you receive approval, use your user name and password to enter the site by
clicking here, or
using the link to the home page, below or at right.
